CMS Launches $50 Billion Rural Health Initiative

Initial $10 billion funding awarded to every state under the Rural Health Transformation program.

The Centers for Medicare & Medicaid Services (CMS) has initiated a significant effort to enhance rural healthcare across the United States through the Rural Health Transformation program. Established by the One Big Beautiful Bill Act in July 2025, this program aims to allocate up to $50 billion over five years to support various rural health activities.

In the latter half of 2025, CMS took crucial steps to implement the program, including the release of a notice of funding opportunity and the distribution of initial funding awards totaling $10 billion to all 50 states. The funding amounts varied, with New Jersey receiving $147 million and Texas receiving $281 million, reflecting the diverse rural characteristics and proposed initiatives of each state. According to the Government Accountability Office (GAO), CMS and merit reviewers assessed state applications based on factors such as the total area in square miles and the percentage of the population living in rural areas.

Monitoring and Future Funding

CMS has outlined a comprehensive plan to monitor the states' use of the program funds. This includes regular communication with states to discuss progress and challenges, reviewing submitted information, and conducting audits and site visits. The agency also plans to measure awardee performance to ensure the achievement of program goals and objectives. Additionally, CMS intends to hold webinars to share lessons learned, improve program outcomes, and foster the adoption of promising practices.

Over the next four years, CMS plans to allocate up to $40 billion of the remaining program funds to states based on their satisfactory progress toward approved initiatives. This progress will be evaluated through various activities, including the achievement of planned milestones. However, if states fail to comply with the terms and conditions of their notices of award, CMS may withhold, reduce, or recover a portion of the program funding.

The authorizing legislation restricts states' ability to appeal or challenge any funding changes made by CMS. However, officials have indicated that the handling of disputes or challenges may depend on specific circumstances, and they are currently determining how to address such issues.

Program Goals and Administration

The Rural Health Transformation program is designed to support activities such as recruiting clinicians to rural areas and providing training and technology to improve care delivery in rural hospitals. The program is administered by CMS through cooperative agreements with states, and federal law grants CMS broad discretion in determining funding allotments.
